Description
As per this comment, it seems that https://github.com/ceph/ceph/pull/39372 broke the support for reverse proxy with a URL prefix.

I couldn't reproduce this issue with haproxy+dashboard. I see that a url_prefix issue was reported a while ago https://tracker.ceph.com/issues/48616, not sure if this is the case here.
I can reproduce it now, got the steps right and have provided the fix too.
