mds: add an equivalent to the OSD OpTracker

Description
Like it says — we want to be able to get information about ops-in-flight and their current status in a lot of different places. Create something that tracks this in a way we can look at and build infrastructure on top of.

Got the OpTracker work rebased on master and pushed for review: wip-optracker, https://github.com/ceph/ceph/pull/610. MDS portion is up next week if nothing else appears (I think there are some external patches that are languishing a bit I might pick up first).
