Had a user with a broken custer going down the wrong debugging path after seeing this in thier OSD logs:
2015-02-23 21:15:42.874339 7f83b437e700 10 filestore(/var/lib/ceph/osd/ceph-11) error opening file /var/lib/ceph/osd/ceph-11/current/70.c0s0_head/DIR_0/DIR_C/DIR_C/DIR_A/default.315696.1\u\ushadow\u420N.bam.2\sQDDQ4F8\uSpkxDhx5mvlpsfjYE89D7VZ.1\u254__head_8FC5ACC0__46_ffffffffffffffff_0 with flags=2: (2) No such file or directory
Probably should document that this is normal behaviour and may not necssarily be related to any cluster issues.

This bug has been judged too old to fix. This is because either it is either 1) raised against a version of Ceph prior to Luminous, or 2) just really old, and untouched for so long that it is unlikely nowadays to represent a live documentation concern.
