mon: removing a running monitor can crash ceph

Description

While rewriting the ceph add/remove monitor documentation (http://ceph.com/docs/master/ops/manage/grow/mon/), I added three monitors to my cluster. The add was successful--i.e., ceph quorum_status showed three running monitors. Then, I followed the directions in the foregoing link and removed monitors c and b respectively. The remove process was performed with the monitor daemons still running. I received a core dump message (see attached).
