Description

A few things:
- clean up the API wrt to setting up a client instance (see #801)
- clean up packaging (/usr/include/ceph/*.h)
- libceph.h should be libceph.hpp
- make sure the wonkier api bits that expose/set layout stuff look sane and reasonably future proof
