The problem as far as I understand is that a versioned object is created, but the ops that creates it is really slow and races with multiple other operations that already execute the olh journal. By the point it tries to update the olh object it's already removed. The fix for it should be trivial (ignore -ENOENT at that point). However, there might be a secondary issue with cleanup of bucket index.
